Report Description:
The purpose of this report is to provide a listing of applicant's contact information and indicators (Subpar, Do Not Hire, Revoked License, Dismissed, Current Employee Status.)
This report is based on 000003 with the addition of indicator fields.
Selection Criteria:
The user may select a date range and application type.
The results are sorted by application modification date and then name.
